Immunizations
2 year olds immunizations:
DTaP/DTP 4 doses
Polio vaccine 3 doses
MMR 1 dose
HepB 3 doses
Varicella 1 dose
Hib 1 to 4 doses
PCV 1 to 4 doses
3 year olds immunizations
No additional shots are required as long as they have all of their 2 year old shots.
However an Updated physical is needed ? checking of the eyes , joints, height , weight etc. There must be a current date for the physical exam
4-year-old immunizations:
DTaP/DTP 5th dose
Polio vaccine 4th dose
MMR 2nd dose
HepB (Should have all 3 doses already)
Varicella 2nd dose
Hib 1 to 4 doses
PCV 1 to 4 doses
